Participating home health agencies are strongly encouraged to include the HHCAHPS fact sheet in the initial patient information packet so that patients are aware of the survey and its importance. HHCAHPS Survey vendors should discuss implementing this fact sheet with their client agencies to help improve patient participation in the survey.

The official HHCAHPS fact sheet is available in English and Spanish.

Agencies do not need prior approval from CMS to use the official fact sheet as long as they do not make any edits other than personalizing the following fields:

• The agency name,
• The vendor’s name,
• The agency’s logo, and
• The vendor’s or CMS logo.
