Survey Vendor Application Process

To become a CMS-approved Home Health Care CAHPS (HHCAHPS) Survey vendor, survey vendors must:

  • Meet the HHCAHPS Survey Vendor Minimum Business Requirements (MBRs) available here.
  • Designate an HHCAHPS Survey Administrator/Project Manager. The designated HHCAHPS Survey Administrator/Project Manager’s role and responsibilities are listed below.
    • Register as your organization’s Survey Administrator/Project Manager on the HHCAHPS website by completing the Vendor Registration Form;
    • Designate another individual within your organization to serve as the backup HHCAHPS Survey Administrator;
    • Remove or grant access to the private side of the website for users in your organization;
    • Serve as the main point of contact with the HHCAHPS Survey Coordination Team; and
    • Notify the HHCAHPS Survey Coordination Team if your role as the HHCAHPS Survey Administrator will no longer be valid and identify a successor.
  • Complete and submit an online HHCAHPS Vendor Application Form available from your customized Dashboard, once logged in.
  • Successfully complete the Introduction to the HHCAHPS Survey self-paced training , a training certification, and all subsequent Update Webinar Training sessions.
  • Complete and submit a Quality Assurance Plan (QAP) after your organization’s first quarterly data submission and on an annual basis thereafter.
