Survey Vendor Application Process

Survey vendors interested in becoming an approved Home Health Care CAHPS Survey vendor will need to meet a number of requirements.

The application and approval processes will consist of the following:

  • Completing and submitting a Vendor Participation Form and demonstrating experience conducting mail, telephone, or mixed mode surveys. The Vendor Participation Form (application form) will be made available as a link on this website, under the Forms for Vendors tab, at least one month prior to each Introduction to HHCAHPS training.
  • Successfully participating in and completing the Introduction to the Home Health Care CAHPS Survey webinar training session and all Update training sessions.
  • Completing and submitting a Quality Assurance Plan, which survey vendors will be required to complete and submit after the first quarterly data submission and on an annual basis thereafter.
