Final Mail Questionnaires and OMB Disclosure Notices Available on HHCAHPS Website

The final OMB Disclosure Notice and mail questionnaire standalone documents for the revised survey has been posted to the HHCAHPS Survey and Protocols webpage in all approved languages. The OMB Disclosure Notice documents are unchanged from the ones posted on 2/19/2026 that contained a “DRAFT” watermark. The mail questionnaire documents now include the OMB expiration date (3/31/2029) and final OMB Disclosure Notice on the survey cover. As a reminder, these survey materials reflect the revised HHCAHPS Survey instrument that is to be used starting with the April 2026 sample month. If your organization has already submitted your revised survey materials with the final OMB information, resubmission is not needed.
