The HHCAHPS Coordination Team has streamlined the placement of survey materials on the HHCAHPS web site by using word and pdf icons of each displayed document. Please click here to view the new survey materials page, or access it by clicking on any of the sublinks under the “Survey and Protocols” menu bar on the HHCAHPS web site.
