The Home Health Care CAHPS Survey Coordination Team has posted the HHCAHPS mail questionnaire and accompanying survey materials in the Eastern Armenian dialect. Survey vendors should work with their mail-only mode client HHAs to determine whether their patients would benefit from an Eastern Armenian language version

The Eastern Armenian survey materials are available at the link below. Please note that all materials are posted as .pdf documents using the Sylfaen font.

The HHCAHPS Team has reposted the pdf version of the Protocols and Guidelines Manual, version 9.0, replacing the appendices listed below, which contain Vietnamese text. The reason for this revision is that the Vietnamese documents included in these appendices did not display properly, as they were converted to pdf without using the required Times VN1 font. 

  • Appendix H (Mail survey cover letters, regular and scannable questionnaires, telephone interview script, proxy interview script),
  • Appendix I (OMB disclosure language), and
  • Appendix J (Supplemental questions).

It is important to note that all stand-alone Vietnamese pdf documents that are posted on the HHCAHPS website under the “Survey and Protocols” tab are correct and display properly.

The HHCAHPS Coordination Team has corrected Appendix B, page B-5, of the HHCAHPS Protocols and Guidelines Manual Version 8.0 so that the field length of each of the ICD-10 "Primary Diagnosis" and “Other diagnosis” data elements is now correctly displayed as "7" rather than "5," as was previously displayed.

This correction has been made to the PDF and MS Word versions of the HHCAHPS Protocols and Guidelines Manuals that are posted on the Survey Materials page. These are linked here:

Please note that no changes were made to the XML appendices, as these already had the correct field length for these data elements.

The HHCAHPS Survey Frequently Asked Questions (FAQs) document for telephone interviewers (included in the HHCAHPS Survey Protocols and Guidelines Manual, Version 6.0 as Appendix K) is now available in Spanish on the HHCAHPS website. This translated set of FAQs was previously included in an earlier version of the Protocols and Guidelines Manual and has not changed. This translation is being made available to promote consistency in the way these questions are handled by bilingual telephone interviewers.

HHCAHPS Survey vendors who administer the HHCAHPS Survey via telephone (phone-only or mixed mode) in Spanish are urged to make these FAQs available to their bilingual interviewers. The FAQs are available at this link:

The following are changes that were recently made to the Chinese-language versions of the Home Health Care CAHPS Survey questionnaires, including the regular and optical scan versions of the questionnaire, the cover letters, Consent to Share Responses question, and to the OMB disclosure notice. These changes were made to ensure consistency across both the Simplified and Traditional versions of the HHCAHPS Survey materials. Note that both the Simplified and Traditional versions of the Chinese-language materials have been updated.

Changes Made to the Simplified Chinese Language Questionnaires

Revised Q3 to be more aligned with the translation in the Traditional Chinese-language questionnaire. Revised Q7 to be more aligned with the translation in the Traditional Chinese-language questionnaire. Revised the response categories in Q29 to be more aligned with the translation in the Traditional Chinese-language questionnaire. Deleted an extra period in Q33. Revised the response...

The Traditional Chinese versions of the cover letters were updated with the addition of the English phrases “Department of Health and Human Services,” “Home Health Compare,” and “Privacy Act” in parentheses following the Chinese translation.

New versions of the Simplified and Traditional Chinese questionnaires have been posted. A revised version of the text for the Consent to Share Responses question in Traditional Chinese has also been posted. The following describes the changes that were made to these revised forms. 

  1. There was an incorrect character for "please" in Question 34 that was removed from both the Simplified and Traditional questionnaires to match the English version.
  2. The Traditional version of the Chinese scannable questionnaire is now correct. The version posted previously was incorrectly labeled as Traditional Chinese but was in fact Simplified Chinese.
  3. The Traditional Chinese version of the Consent to Share Responses question had an extra character in the question text which has since been omitted.

The Coordination Team posted the HHCAHPS mail questionnaire and accompanying survey materials in Traditional Chinese on May 9, 2011. The Traditional Chinese language version of the HHCAHPS Survey Questionnaire and related survey materials do not replace the Simplified Chinese language version of the instrument and survey materials. The Traditional Chinese language versions of these documents are provided for use with patients who read Traditional Chinese rather than Simplified Chinese. Vendors should work with their client HHAs to determine whether their patients who need a Chinese language version of the instrument need the questionnaire in Simplified or Traditional Chinese, and use the applicable version.

Effective immediately, all survey vendors conducting HHCAHPS mail surveys must display the HHCAHPS survey OMB control number (0938-1066) on the front cover of the HHCAHPS questionnaire or, if there is no cover, on the first page of the questionnaire. This change is necessary to be in compliance with OMB regulations.

The Protocols and Guidelines manual will be updated to reflect this change and relevant pages will also be updated on the HHCAHPS Web site. The following changes will be made in the Protocols and Guidelines Manual.

Chapter V, page 48

The Office of Management and Budget (OMB) number shown in Appendix H must be printed on the questionnaire cover. If there is no cover, then the OMB number must be printed on the first page of the questionnaire. Chapter VII, page 72

The Home Health Care CAHPS (HHCAHPS) Survey Coordination Team has posted revised versions of the HHCAHPS proxy telephone scripts for all languages in which the HHCAHPS Survey can be administered by telephone mode (English, Spanish, Russian, and Vietnamese). A change was made to Q17 of the proxy telephone interview script to remove the sample member name fill, which brings this question in line with the way Q17 is worded in the mail survey. CMS has decided that the wording of this question in the telephone scripts (for both the sample member and the proxy respondent) should be exactly the same as it is in the Mail Survey questionnaire (with no fills).

Additionally, the translation of the word “MISSING” (as part of the MISSING/DK response option that follows every question) in the Russian-language version of the telephone scripts for the sample member and the proxy respondent was revised slightly. Please note that “MISSING” is not a word that is ever read to respondents; therefore, this change should not...

The following are changes that were recently made to the Spanish-language version of the Home Health Care CAHPS mail survey, scannable survey, telephone script and proxy respondent script.

  1. Language text of the instructions in bullet #2 of the Mail survey was changed to match the text in the Scannable survey.
    • Previous language was: A veces hay que saltarse alguna pregunta del cuestionario. Cuando esto ocurra, verá una flecha con una nota que le indicará la siguiente pregunta a la que tiene que pasar.
    • New Language is: A veces se le indica que debe saltarse algunas preguntas de esta encuesta. Cuando ocurra, verá una flecha con una nota que le indica qué pregunta es la siguiente, de esta manera:
  2. Q3 formatting, changes were made for consistency in bolding.
    • Telephone script- bolded “le” and removed bolding from “sobre”. Changed from “le habló sobre” to “le habló sobre.”

Spanish questionnaire and telephone scripts have been updated to ensure consistency of language between the two documents. Survey vendors currently fielding the Spanish instrument should wait until the beginning of a new sample month before making any updates to the materials.
