Survey vendors interested in becoming an approved Home Health Care CAHPS Survey vendor will need to meet a number of requirements.

The application and approval processes will consist of the following:

• Completing and submitting a Vendor Participation Form and demonstrating experience conducting mail, telephone, or mixed mode surveys. The Vendor Participation Form (application form) is available as a link on this website, from the Vendor Dashboard, once a vendor has requested and obtained login credentials.
• Successfully participating in and completing the Introduction to the Home Health Care CAHPS Survey webinar training session and all subsequent Update training sessions
• Completing and submitting a Quality Assurance Plan, which survey vendors will be required to complete and submit after the first quarterly data submission and on an annual basis thereafter.
