Placement of OMB Control Number on HHCAHPS Mail Survey Questionnaires

Effective immediately, all survey vendors conducting HHCAHPS mail surveys must display the HHCAHPS survey OMB control number (0938-1066) on the front cover of the HHCAHPS questionnaire or, if there is no cover, on the first page of the questionnaire. This change is necessary to be in compliance with OMB regulations.

The Protocols and Guidelines manual will be updated to reflect this change and relevant pages will also be updated on the HHCAHPS Web site. The following changes will be made in the Protocols and Guidelines Manual.

Chapter V, page 48

• The Office of Management and Budget (OMB) number shown in Appendix H must be printed on the questionnaire cover. If there is no cover, then the OMB number must be printed on the first page of the questionnaire.

Chapter VII, page 72

• The Office of Management and Budget (OMB) number shown in Appendix H must be printed on the questionnaire cover. If there is no cover, then the OMB number must be printed on the first page of the questionnaire.

In most cases, it is a good idea to print the OMB No. in the upper-right corner of the questionnaire.

Note that the OMB disclosure statement must also be printed on the questionnaire on either the questionnaire cover page or the last page. If necessary, the disclosure statement can also be included in the mail survey cover letter.
